Patient Online Pre-Registration Instructions
Wayne Memorial Hospital’s online patient pre-registration process is a handy, time-saving option that allows you to pre-register at your convenience. We recognize the confidential nature of the information you submit online. For this reason, you will be transferred to a secure page to complete the pre-registration process.
Before you begin the pre-registration process, you will need the following:
- Insurance ID card information
- Procedure Date & Time
- Type of Procedure (e.g. mammogram, xray,lab,etc.)
- Personal Contact Info
- Physician Order

All patients must stop by the Admissions Department in the main lobby to complete the registration process.
